Licensed Practical Nurse - Patient Aligned Care Team

The Licensed Practical Nurse (LPN) performs assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran working for the community based clinic and is a key member of the PACT and multidisciplinary team working within the North Florida/South Georgia catchment area located in Tallahassee Outpatient Clinic (OPC), Jacksonville, Middleburg, Saint Augustine in FL or Valdosta, GA.The LPN is able to provide clear communication, build relationships with patients, families, and between team members to deliver comprehensive care to veterans. The goal of PACT is to provide the right service at the right time, rather than waiting for the patient to self-identify and seek out healthcare. Population management activities include preventative services and immunizations; monitoring patients at high risk for chronic diseases enabling identification of gaps in clinical care; and using strategies for improving health care outcomes for the defined patient cohort.The LPN/L VN performs a wide range of clinical care and supportive duties through the PACT care delivery model. The nurse must demonstrate basic practical nursing knowledge and the skills necessary to carry out practical nursing care assignments within the scope of their education and licensure. In addition, employees at this grade level is able to perform task such as assist with the Annual Flu Clinic, participates in community outreach, mock survey properness for Joint Commission.Other functions or scope of assigned duties include, but are not limited to:Uses the Computerized Patient Record System (CPRS), and Bar-Coded Medication Administration (BCMA), software per policy.The LPN/LVN actively assists with the care coordination and communicate needs of high-risk patients to the team.Facilitates clinic check-out and follow-up visits by reviewing post-visit needs.Enters orders using the approved PACT Nursing Protocols (Hold to Sign) in response to identified patient needs and orders may include labs, procedures such as EKGs and Chest X-Rays.Performs a wide range of clinical and administrative functions on a daily basis including, but not limited to, completing clinical reminders, conducting patient data intakes, answering secure messages, multi-level patient tracking, schedule preparation (scrubbing), and reinforcing patient education.Assists with diagnostic procedures, such as biopsies, which include preparing the patient, setting up sterile field and instrumentation, and monitoring the patient's condition when applicable to the outpatient clinic setting.Recognizes, reports, and documents significant, as well as, subtle changes in a patient's physical condition ad/or emotional condition or changes to higher level independent provider such as ARNP, RN, MD, or assigned Supervisor.Provides instructions to patients with pre- and post-procedures, diagnostic test, and take an active role in promoting Whole Health practices.Total Rewards of a Allied Health ProfessionalWork Schedule: Full-Time, Monday - Friday, 8:00am - 4:30pm, 40 hours per week.Compressed/Flexible: Not available.Telework: Not available.Virtual: This is not a virtual position.Functional Statement #: Licensed Practical/Vocational Nurse, FS #51827F
