Assistant Resident Care Coordinator
Full-Time, 40 hours a week. 8:30am-5:00pm
Job Description
Assists the Director of Resident Care in maintaining Nursing Service philosophy, objectives, and standards of practice to ensure standards of excellence for the residents. In conjunction with other disciplines, endeavors to assure that the multi-disciplinary plan of service is maintained.
EDUCATION, EXPERIENCE, CERTIFICATION AND LICENSURE REQUIREMENTS
Certification as a Licensed Practical Nurse by the Commonwealth of Virginia required.
CPR/AED certification required.
4+years of licensure as a practical nurse is required
2+ years of supervisory experience preferred.
Job Duties
Assists with 24-hour clinical services in memory support and/or assisted living within scope of practice.
May assist with the education of clinical matters in the community and as a resource to the clinical team for regulatory compliance, general operations, infection control, and team member education.
Helps ensure all clinical aspects of transitioning residents are met.
May help prepare clinical work schedules, make recommendations regarding personnel status and appropriate staffing levels required within operating guidelines.
May assist the Director of Resident Care in making daily rounds for supervision of service, resident status, environment, and safety and submit reports to the Director of Resident Care or in their absence the Executive Director.
Helps ensure, within scope of practice, that clinical practice is being performed through established routines and in compliance with community procedures and governmental regulations.
May be assigned to maintain inventory control of supplies and submit purchase orders to the Executive Director.
Helps coordinate nursing practices toward restoring and maintaining a holistic approach to meet residents’ entire physical, emotional, behavioral, psychological, familial, spiritual, social, social, and nutritional needs.
Assists with UAI assessments and ensuring the service plans are written and revised according to State and Federal regulations for all residents.
Assists in ensuring that all operating policies and procedures are followed and comply with Federal and State regulations.
May act as liaison between Physicians, Rehab, Pharmacy and Nursing team members.
May be assigned to coordinate ongoing in-service education program for the nursing team members. Attends mandatory in-service and completes Relias Learning in-services assigned.
Effectively coordinates nursing services with medical, rehabilitation, social services, dining services and activities programs.
Evaluates environmental safety and recommends changes when applicable.
Actively participates in Resident Care Conferences, Infection Control Committee and Continuous Quality Assurance/Performance Improvement (QAPI), as applicable.
May assist in interviewing and assessing within scope of practice applicants for admissions into the community.
Oversees all software programs utilized by the clinical team.
All other duties as assigned.
Qualifications
Practical knowledge of theories, practices, and procedures within nursing. Has conceptual and practical experience.
Strong verbal and written communication skills required.
Acquired specialized knowledge of disease processes and treatments.
Ability to hear, see and smell.
Ability to read, write and communicate using the English language,
SUPERVISION and Decision Making EXERCISED
Receives limited supervision – works mostly independently, using independent judgement and receives little guidance.
Decision-making can be difficult. Most alternatives that are considered are covered by standard operating procedures and are of comparable value. Decision making sometimes requires making predictions regarding the long-term impact of the decisions.
Problem solving is sometimes complex in nature and utilized when needed. Ability to use expertise to resolve issues.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
May include frequent lifting (up to 50lbs).
Extensive walking, bending, stooping, standing.
Ability to work in a fast-paced environment with multiple tasks.
Ability to work inside and outside to effectively make rounds daily throughout the facility and to respond to problems and emergencies.
Universal staffing. Flexible schedule as working evenings, weekends, holidays, and during inclement weather are required to meet the needs of the job.
Exposure to the sights, sounds, and odors of a typical heath care environment including persons with dementia.
