Speech Language Pathologist
Job Summary:
We are seeking a dedicated Speech Language Pathologist to join our team. The ideal candidate will work with patients to diagnose, assess, and treat speech, language, cognitive-communication, voice, fluency, and other related disorders.
The Provider shall perform the following work:
I. Perform services as a New Mexico licensed Speech Language Pathologist in good standing for geriatric resident population at the client in Santa Clara, New Mexico.
II. Possess a Certificate of Clinical Competence or Clinical Fellowship Year with speech, language, hearing, cognitive and swallowing disorders.
III. Provide services on an as-needed, as required basis in accordance with a periodic work schedule and long-term-care regulations.
IV. Have a minimum of one-year experience in physical disabilities rehabilitation.
V. Have up-to-date Occupational Safety and Health Administration mandated training on blood-borne pathogens.
VI. Have up-to-date Hepatitis vaccine or document of declination, have recent two-step Tuberculous test or chest X-Ray as applicable.
VII. Provide proof of liability insurance and current copies of professional licensure.
VIII. Compliance required with all applicable requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
IX. Coordinate and report hours worked to facility Rehabilitation Director.
X. Provide timely and accurate documentation of assessments, findings, goals and recommendations for patient medical record, care plan or MDS per regulatory compliance and facility policy and procedure.
XI. Provide training and instruction to appropriate staff responsible for Speech Language Pathologist related plans of care. These include but are not limited to swallowing improvement techniques, diet texture modification, cognition, and communication as per facility policy and procedure.
XII. Maintain effective communication and have an active role with medical staff, nursing, dietary and rehabilitation clinical team.
XIII. Participate in comprehensive Facility orientation at required intervals prior to work activities as notified.
Qualifications:

- Valid state license to practice as a Speech Language Pathologist.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
