Registered Nurse (RN), Home Health Evening Triage

AS A HOME HEALTH EVENING REGISTERED NURSE (RN), you'll be responsible for triaging all patient and patient family phone calls for the Agency between the evening hours Monday through Friday, assessing the nature of the call and taking appropriate action. In addition, this RN is responsible for an assignment of patients for the evening hours, responsible for identifying patient/family needs utilizing the nursing process of ongoing assessment, planning, intervention, implementation and evaluation, and is responsible for identifying patient/family needs and providing quality care/services in accordance with the attending physician's orders, plan of care and Agency policies and procedures.
