RN, Community Health Nurse (Home Care) - Full Time

Job Description Join Our Team as a PRN Registered NurseWe invite you to become a part of our dynamic team as a PRN Registered Nurse! This position is available for day shifts from 6:00 AM to 6:30 PM.Our organization is recognized for excellence in healthcare and has been honored among the Best Nursing Homes by U.S. News & World Report.Compensation and ScheduleHourly Pay: $39.59 - $46.55, depending on experience.Schedule: PRN, day shifts (6:00 AM - 6:30 PM), with occasional weekend shifts required.Key ResponsibilitiesThe Registered Nurse will employ the nursing process to plan and deliver direct care to assigned residents. Responsibilities include:Planning, directing, and executing shift routines within the designated nursing unit.Observing, guiding, and assessing the activities of staff as the designated leader, in accordance with the Nurse Practice Act.Assuming Building Charge responsibilities as assigned by the Director of Nursing or their designee.QualificationsCurrent Registered Nurse licensure in Colorado.Experience in long-term care is preferred.Strong problem-solving skills with the ability to make independent decisions using initiative and discretion.Customer service-oriented with knowledge of effective service techniques.Clinical proficiency in performing physical and cognitive assessments of residents.Competence in using computers and electronic devices for tasks such as timekeeping, documentation, and in-service training.Familiarity with Antibiotic Stewardship, Infection Prevention, Infection Control, and OSHA guidelines.Leadership and CommunicationAs a leader in your unit, you will:Guide and support Aides, STNAs, TMAs (if applicable), interdisciplinary teams, and management.Maintain open communication with supervisors, colleagues, and incoming shifts.Conduct rounds to assess residents' physical and psychosocial well-being.Assign tasks to staff to meet residents' needs effectively.Ensure that all personnel fulfill their responsibilities during your shift.Participate in scheduled nurses' and team meetings.Initiate disciplinary actions when necessary based on staff performance.Report any occupational exposures in line with facility policies.Present concerns and suggestions for improvement to facility leadership and relevant committees.Resident Care DutiesIn your role, you will ensure that residents are treated with dignity and respect by:Reporting any suspected abuse or neglect immediately, following company policy.Administering nursing techniques to promote residents' comfort and well-being.Being knowledgeable about medications and reporting any side effects or concerns to physicians.Maintaining accurate and timely documentation as per facility policies.Following established routines for monitoring vital signs, weights, and administering medications.Communicating changes in residents' conditions to physicians and families.Adhering to emergency protocols in case of a 911 call.Respecting all Advance Directives, including DNR/DNI/CPR/Comfort Care protocols.Documentation and EvaluationYour responsibilities will also include:Contributing to the development and updating of residents' care plans.Evaluating the performance of staff under your supervision.Completing routine charting duties in accordance with established policies.Accurately transcribing physician orders and communicating necessary information to other departments.Documenting and reporting all incidents involving residents, visitors, and staff.Providing ongoing assessments of residents as required and documenting these assessments appropriately.Professional DevelopmentWe encourage your growth by:Participating in scheduled nurses' meetings.Engaging in employee performance evaluations and staffing recommendations.Attending required in-services and completing assigned online training modules.Joining various committees and meetings to enhance interdisciplinary collaboration.Work EnvironmentLocated at the base of the picturesque Grand Mesa in Eckert, CO, our facility offers a supportive community atmosphere amidst stunning natural beauty.
