Physical Therapist (PT) – Medicare Home Health (Full-Time & Per Diem)Activa Home Health is seeking experienced and clinically strong Physical Therapists (PTs) to join our team supporting both our Medicare Home Health patients and when needed our Hospital at Home cases. We are hiring for full-time and per-diem roles across Indian River, St. Lucie, and Martin Counties.This role is ideal for therapists who thrive in acute care, home health, and high-acuity environments, and are passionate about restoring mobility, improving safety, and supporting patients' independence at home.Position SummaryThe Physical Therapist provides skilled, patient-centered therapy services for both acute-level (Hospital at Home) and Medicare home health patients, focusing on mobility, fall prevention, functional independence, and successful recovery outcomes.Key ResponsibilitiesPerform comprehensive PT evaluations, including:Strength, balance, endurance, and functional mobilityGait analysis and fall risk assessmentTransfers, bed mobility, and stair negotiationActivity tolerance and cardiopulmonary responseDevelop and implement individualized, evidence-based treatment plansAddress functional decline related to acute illness, injury, or recent hospitalizationProvide gait training, balance interventions, and mobility progressionEducate patients and caregivers on:Safe mobility and transfer techniquesFall prevention and home safetyUse of assistive devices and equipmentMonitor patient progress and communicate changes with the interdisciplinary teamComplete accurate and timely documentation, including OASIS assessments (preferred)Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce readmissionsRequired QualificationsActive Florida Physical Therapist (PT) licenseDegree from an accredited Physical Therapy program (DPT preferred)Current CPR certificationValid driver's license and reliable transportationStrong clinical reasoning, communication, and time-management skillsPreferred ExperienceMedicare home health experienceOASIS experience preferredAcute care or hospital-based physical therapyHospital at Home, home health, or transitional care experienceExperience treating medically complex patientsFamiliarity with EMR systemsWhy Join Activa Home Health?Flexible scheduling with full-time and per-diem opportunitiesCompetitive compensationSupportive leadership and collaborative clinical environmentOngoing education and professional growth opportunitiesOpportunity to work across innovative care models (Home Health + Hospital at Home)Apply TodayIf you're a Physical Therapist looking to apply your acute care and home health expertise in a progressive, patient-focused setting, we encourage you to apply and join our growing team serving Indian River, St. Lucie, and Martin Counties.
