Registered Nurse - Clinic Care Coordinator (1.0 FTE)

DescriptionWestern Wisconsin Health is seeking a full-time Registered Nurse (1.0 FTE) to work with our Primary Care team of Providers and Staff. The Rural Health Clinic (RHC) Registered Nurse Care Coordinator's primary function is to guide patient through the health care system by assisting with access to care challenges, developing relationships with service providers, tracking interventions, outcomes and follow ups. Follow patients through the continuum of care including post discharge follow up. The RHC RN Care Coordinator collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. All of this is done to support the organization's mission while living our values.ESSENTIAL DUTIES AND RESPONSIBILITIESPerforms assessments and utilizes organizational protocols to triage and direct patients to the appropriate level of care;Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response;Serves as primary contact for clinic patients and their families seeking information and/or requesting additional services;Develops patient education programs and tools;Shares data with WWH committees and other groups which demonstrate outcomes and performance improvement activities;Assists in identifying opportunities for process improvement;Maintains appropriate infection control standards and precautions;Engages in continuous learning with an emphasis on assigned area(s) of service;Stays up-to-date on new technologies;Demonstrates awareness, understanding and respect of cultural diversity and patients of all ages and socioeconomic groups; and,Other duties as assigned.RequirementsQUALIFICATIONSWestern Wisconsin Health expects employees to understand and to incorporate the values of our organization in their day-to-day practice.
