--Seeking a BC/BE Family Medicine, Internal Medicine or Geriatric trained physician to join a non-profit organization--This program of all-inclusive care is designed to allow participants of the program to remain in their own homes and communities while providing their caregivers with much needed relief and emotional support--Services Include: All primary and specialty physician services, Adult Day Health Center services, Social services and family, caregiver support, Medical equipment, Recreational therapy, Physical and occupational therapy services, Personal care services in the home, Transportation to and from the center and medical appointments, Nutritional counseling Home delivered meals, Medications--The physician would be housed in a brand new health center and clinic and would occasionally need to go and do assessments of the patients in their homes or in the nursing home setting when necessary--There will be Advanced Practitioner oversight--Hours are Monday through Friday, 8 am - 5 pm with minimal rotating call expectations--Compensation includes: Base Salary, Production and Quality Bonus Opportunity, Healthcare Benefits, CME Allowance, Malpractice, Retirement
