Housing Case Manager & Social Services Coordinator Duties and Responsibilities Assists clients in achieving their housing goals Willingness to work with clients with serious mental illness Develops a person-centered service plan for each client Assesses each client's needs, with an emphasis on strengths and barriers related to getting & keeping their housing Works with property management staff to help clients maintain their housing Identifies strategies that will help clients maintain their housing and strengthen their recovery Coordinates/facilitates speakers, groups or events that foster community, identify helpful resources, and reinforce wellbeing Supports clients in understanding the basic expectations of being a tenant, lease requirements, household maintenance, etc. Collaborates with landlords and other community partners, advocating for client needs and rights.
