The PT will evaluate patients, develop individualized treatment plans, and provide therapy services to help patients improve mobility, reduce pain, restore function, and increase overall independence. This role involves working with patients recovering from injury, surgery, illness, or other physical limitations.ResponsibilitiesEvaluate patients and develop individualized physical therapy treatment plansProvide therapeutic exercises, manual therapy, gait training, balance training, and functional mobility interventionsMonitor patient progress and modify treatment plans as needed to achieve rehabilitation goalsEducate patients and caregivers on exercises, injury prevention, recovery plans, and home exercise programsMaintain accurate and timely patient documentation in accordance with facility and regulatory standardsCollaborate with physicians, nurses, therapists, and interdisciplinary healthcare teams to coordinate patient careAssist with discharge planning and recommendations for continued therapy servicesMaintain a safe, organized, and professional therapy environmentDeliver compassionate, patient-centered care while promoting positive patient outcomesRequirementsRequired: Graduate of an accredited Physical Therapy program (DPT preferred)Required: Active PT license in state of practice or ability to obtainPreferred: Previous physical therapy or healthcare experienceStrong communication, organizational, and interpersonal skillsAbility to work effectively in a collaborative healthcare environmentTowne U.S. LLC Job ID #18275226.
