About the OpportunityHy-Vee Health Exemplar Care is operating a 24/7 Urgent Care.We are seeking Advanced Practice Providers (Nurse Practitioner / Physician Assistant) to join our urgent care team. Full-Time shifts, 36 hours a week, rotating weekends and holidays.Core activities of the rolePerform a medical history and physical examination on patients of all ages.Personalize the medical history and examination to match the reason for the clinic encounter (acute issue, chronic issue, or health maintenance).Appropriately utilize diagnostic tests to supplement the medical history and exam.Interpret medical histories, physical exams, and diagnostic tests to develop the best treatment plan.Implement and manage treatment plans.Prescribe medications and treatments.Perform medical procedures as necessaryCoordinate referrals to other healthcare professionals and medical facilities.Counsel and educate patients about their tests, medications, and treatment plans.Counsel and educate patients on healthy lifestyle and how to make lifestyle changes to improve health.Be familiar with and utilize community resources to assist patients with their physical, mental, and social needs.Work with colleagues to ensure that Hy-Vee Health Exemplar Care patient needs are addressed in a timely manner.Timely documentation of clinic encounters and patient interactions in the electronic health record.Competence to practice to the fullest extent allowed by your state licensure.Adhere to the policies and procedures of Hy-Vee Health Exemplar Care.Adhere to government rules, regulations, and laws.Other duties as assignedBenefits (other total comp package)We offer competitive compensation, generous 401K, short- & long-term disability, basic life insurance, PTO, Direct Primary Care Membership, CME allowance, optional health, dental and vision.Required Qualifications (Education and Experience)Experience preferred.State license to practice as an NP/PA.Physical RequirementsProlonged periods of sitting at a desk and working on a computer.Must be able to push, pull, lift, lower, or other movements at 25 ? 50 pounds at rare times with or without assistanceMust be able to push, pull, lift, lower, or other movements at 10 ? 25 pounds at semi-often timesMust be able to push, pull, lift, lower, or other movements at 0 ? 10 pounds frequentlyEnvironmental RequirementsModerate levels of sound, consistent amount of moving around between rooms required, in a temperature-controlled environment, and consistently working with the general public.Core ValuesAt Hy-Vee Health Exemplar Care weFIERCELY CAREDO RIGHT BY PEOPLEDO WHATEVER IT TAKESALWAYS IMPROVEEEO StatementWe believe in the value of diversity within our workforce and is an equal opportunity employer.
