Fosters continuity of care across the continuum of care.Provides nutritional care and education to patients/family in assigned area in a timely fashion according to standard policies and procedures.Works closely with interdisciplinary team members to provide quality nutrition care to patients.Teaches Cardiac and Pulmonary Rehab classes, community education classes, and staff education.
