We are a non-profit organization which includes Diakon Child, Family, and Community Ministries and Diakon Affordable Housing.General Responsibilities:Provide full range of in-depth social work services, including case management to client population in accordance with state regulations and agency policies.Employs a range of social work interventions with client families to facilitate accomplishments of goals listed in the Service Plan.Makes initial case assessment and ongoing assessments.Provides case management and face-to-face services as described in Service Plan, to meet stated client goals. Education/Training: A bachelor's degree in social work, BSW, normally required. Experience: A minimum of two (2) years' experience in a related child welfare or human service position normally required.
