A company is looking for a Case Manager Registered Nurse (Remote, Texas). Key Responsibilities Facilitate delivery of healthcare information and determine eligibility for benefits Develop and implement health strategies and programs to promote member wellness Coordinate care through assessment, planning, and advocacy for members' health needs Required Qualifications Unrestricted and active TX/Compact RN licensure Minimum 3+ years of clinical practical experience, preferably with Medicare members Minimum 2+ years of case management, discharge planning, or home health care coordination experience Associates degree or equivalent experience required; Bachelor's degree preferred
