If yes, join the team at Koinonia Family Services as a part-time ECM Care Coordinator serving clients throughout the Greater Sacramento region, including Sacramento County and Placer County. As a Care Coordinator, you'll play a key role in connecting individuals to services and resources tailored to meet their unique needs. Your responsibilities will include conducting assessments, developing individualized care plans, coordinating services and referrals, and providing education to help individuals make informed, healthy lifestyle choices.Opportunity HighlightsEnhanced Care Management is a statewide Medi-Cal benefit available to individuals with complex needs. The Care Coordinator will lead the assessment, care plan development and ongoing care management of eligible participants under the supportive guidance of an ECM Supervisor. Our program specifically targets foster youth and other at-risk youth populations, but may also include their family members. The Care Coordinator utilizes motivational interviewing to engage individuals to support them in better managing their chronic conditions. The Care Coordinator actively engages individuals to remove barriers to health, social services, and behavioral health care.The Care Coordinator Opportunity IncludesA job with a purpose; help support youth and families in needHourly pay rate: $24.00 per hour to start, or higher with prior ECM experienceBenefits include paid time off (PTO), paid holidays, and participation in our 401k planA variety of shifts that work with your schedule (afternoons/early evenings)Remote work available between in-person sessionsWork with a great team of people who care about each other and care about our mission to help youth and families find hope and healingMileage reimbursement for miles driven for businessThis is a part-time role offering 25-28 hours per week.Key ResponsibilitiesUnder the leadership of the ECM Supervisor, you will work with the ECM team to provide services to youth and families as follows:Conduct assessments of individuals' needs and formulate individualized care plansEnthusiastically educate and enroll clients about services, and motivate them to engage in services.Meet with individuals in their homes to facilitate servicesOversee and ensure the seamless coordination of services, including physical, mental, dental care and social servicesAct as the primary liaison between individuals and their healthcare providers, and social servicesStay informed about community resources and actively refer individuals to appropriate services as neededMaintain regular contact with clients to review and update care plans, and monitor clients' progress and outcomesAdvocate for the needs and rights of clients within healthcare and social service settingsProvide clients and families with resources and guidance on making informed health and lifestyle choicesDocument services according to ECM program standardsWork closely with interdisciplinary teams, community partners, and stakeholders to enhance service delivery.QualificationsA strong desire to help people in need achieve successful outcomesMinimum 2 years of case management (or related) experienceKnowledge of community resourcesExcellent interpersonal and communication skillsComfortable talking to people and motivating them to seek servicesAbility to collaborate with interdisciplinary teamsExperience with high-needs populationsProficiency in electronic health records a plusA high degree of integrity, ethics and confidentialityDemonstrated ability to work independently and maintain productivity with minimal supervisionCultural sensitivity and awarenessValid driver’s license and auto insuranceWe are seeking candidates who have at least two years of related work experience in social work, case management, community engagement or customer service. High school graduate or GED required. The nature of these responsibilities necessitates the ability to drive to ensure timely, reliable, confidential and effective support for the youth and families we serve.
