Make a difference in the lives of children and families!Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our CLTS Waiver Program team in Walworth County!The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to live safely in their homes and communities.
