LICENSED PRACTICAL NURSE LPN PART TIME -11p-7a Position Summary The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program. Major Responsibilities Maintains nursing documentation and medication records  Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR  Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc  Assesses residents’ health and emotional needs  Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director  Maintains confidentiality, including keeping voice low and not talking about residents in public areas  Other duties as assigned    Job Qualifications Current PA RN/LPN licensure Current CPR/First Aid certification Valid Driver’s License Act 33/34 clearances FBI clearance as needed All trainings required by licensure Must complete a Physical, TB test, and Hep B titer.
