Louisville, KY 40205ShiftSalary Shift (United States of America)Job Description SummaryUofL Health is a fully integrated regional academic health system with five hospitals, four medical centers, nearly 200 physician practice locations, more than 700 providers, the Frazier Rehab Institute and Brown Cancer Center.With more than 12,000 team members—physicians, surgeons, nurses, pharmacists and other highly skilled health care professionals—UofL Health is focused on one mission: delivering patient-centered care to each and every patient each and every day.Job DescriptionThe director will assess community behavioral health needs, provide input for facility strategic planning, business development, community outreach and maintain ongoing communication with key facility referral sources to promote information about Peace’s services; provide customer service with coordination of patient care; ability to discuss treatment/recommendations with referral sources.Essential FunctionsReviews referral source data and track trends from referral sources in territoryInitiates face to face contact with referral sources throughout the assigned region by conducting outreach visits to their agenciesMaintains positive referral source relationships in targeted regionsResponsible for processing referral source complaints working directly and appropriate administrative personnel in developing strategies to resolve concerns and improve servicesLeads monthly department meetings.Works directly with UofL Health’s marketing department to develop promotional materials, participate/coordinate media opportunities, write and coordinate blogs, special media posts, website content, etc.Participate in and represent Peace Hospital on community coalitions and committees.Assist in coordinating Peace Hospital’s annual fundraising event.Seek new business opportunities for growth, develop memorandum of understanding and assist with grant proposals.Other FunctionsAbides by the Standards of Conduct in addition to all applicable state and federal regulatory requirementsDemonstrates a commitment to the presentation of a professional image in the workplace with regard to dress, personal appearance and hygieneAbides by UofL Health policies regarding attendance and timeliness as appliesAchieves excellence through teamwork by working with others to provide desired outcomesExhibits willingness to learn/perform tasks outside the scope of his/her regular job duties and/or work additional hours/shifts as necessary to meet customer needsCompletes all assigned Annual Competencies and KNOW Modules by due date.
