LPN /Wellness coordinator

Job Summary Job description We are searching for a Health and Wellness Coordinator for our small personal care home serving older ladies and gentlemen, most with a diagnosis of ID. Duties include but are not limited to: Oversee medication administration program as well as pass medication during scheduled shifts Oversee overall health for older population of ladies and gentlemen with a diagnosis of ID/ DD Complete indicated assessments (i.e. monthly, weekly, urgent etc) Schedule and complete all indicated physician, specialist visits/ diagnostic as indicated by individual need. Both off site and virtual. Company vehicles available. Noting of completed appointments ensuring indicated follow up is implemented. Qualifications: Current PA Nursing license (LPN or RN) Experience with elder care, psych and/ or ID Ability to work independently and as part of a team Strong organizational skills Desire to have both hands on care as well as administrative duties Dependable and passionate about helping others Driver's license and good driving record Note: This job description is not intended to be all-inclusive.
