We make it fun for patients to stay on top of their medications, help them address medication challenges, and empower them to improve their health by addressing barriers to adherence related to social determinants of health.Job Overview – Clinical Pharmacy ManagerWe are seeking a highly motivated and experienced Clinical Pharmacy Manager to lead our team of clinicians and healthcare workers in providing exceptional patient care. You will play a key role in ensuring the efficiency and effectiveness of our pharmacy services, in particular comprehensive medication reviews (CMRs), and provider engagement strategies. This role requires a strong leader who can inspire and guide their team, develop and implement operational plans, and collaborate across departments to enhance our services and improve patient outcomes. This is a Manager/Doer role that requires one to be deeply involved with delivering direct clinical consultation with patients while also having a voice within broader strategic executive planning. The role will increase conversion into our clinical and linkage to care programming.Responsibilities Include:Provision of Pharmacy Services (70%)Conducts patient outreach and manages inbound calls to provide pharmacist services to patients as part of a Scene programDeliver MTM services to assess the appropriateness of all medications to optimize medication therapy and improve clinical outcomesEngage with patients to provide defined services such as comprehensive medication reviews, targeted medication reviews, medication reconciliation, prior authorization review, and other clinical servicesAssess the use of prescribed medication therapy for unnecessary medications, additional medication needs, inappropriate dosing, adverse drug reactions, ineffective medications, and adherence issuesDocument all clinical recommendations, responses to therapy, and patient care notes in a clear, concise, timely, and professional mannerSend clinical recommendations to provider offices and follow up on the responses to ensure recommendations are enactedCoordinate with Scene Team, including nurses, health coaches, management, product managers, and client-facing staff as neededTeam Supervision (20%)Hire, train, coach, and evaluate direct reportsImplement Quality Assurance Tool and conduct QA weekly, ensuring ongoing coaching of team members to achieve program standardsSet schedules for pharmacists and pharmacy vendors to ensure coverage for pharmacy service implementation and appropriate staffing to deliver high-quality patient careConduct clinical training and refresher presentations as neededDevelop and Implement Processes (10%)Identify and implement innovative process and workflow improvementsCollaborate with Product and Experience teams to enhance patient and customer supportAlign clinical service offerings with market needs and company value propositionsEnsure proper documentation of servicesRequired QualificationsActive Pharmacist License with at least five years of pharmacy practiceAbility to obtain licensure in additional states as neededStrong Telephonic Skills: Must be able to serve patient populations, including individuals insured by Medicare and Medicaid, in a clear and fluid mannerExcellent communication skills with the ability to develop patient rapportAbility to covert participants into Scene Health services, including pharmacy services and additional clinical interventionsExceptional listening and interpersonal skillsAbility to work independently as well as in collaboration with a virtual/decentralized teamAbility to preserve/protect confidential informationSelf-motivated and able to multitaskProficient in Microsoft Teams, Outlook, Word, Excel, and other programsAlignment with Scene’s Cultural BlueprintPreferred QualificationsMedically fluent in English and SpanishExperience working in digital health programs and/or working for a health startup Experience working with hospitals or health plans on quality improvementFluency in Salesforce and additional documentation platformsSales or call center experience is preferred but not requiredYou’ll love this role if you are:Strong in problem-solving, communications, collaboration, research, and analyticsResults-oriented and thoughtful in your work styleAble to work autonomously from idea generation through project executionHighly organized, efficient, able to prioritize, and manage a wide range of responsibilitiesAble to consistently deliver high-quality work on time and in a manner that makes everyone want to work with youBenefitsThe biggest perk is that you will be working on a game-changing solution in healthcare with people who are talented, motivated, and passionate.
