In this role, you will provide case management services to clients by assessing needs, coordinating resources, and developing personalized care plans. The Case Manager will work closely with clients and their families to ensure they have access to integral support services, monitor progress, and make adjustments to care plans as necessary. Responsibilities also include maintaining precise documentation and collaborating with a multidisciplinary team to improve client outcomes.QualificationsBackground in Social Work and Case ManagementStrong Interpersonal Skills for building relationships with clients, families, and team membersExperience in providing Case Management Services and support coordinationA solid understanding of Psychology and mental health conceptsOrganizational and problem-solving skillsBachelor’s degree in Social Work, Psychology, or a related field; Master’s degree preferredValid driver’s license and reliable access to transportationWorking knowledge
