Summary:
We are seeking a dedicated and experienced Licensed Master Social Worker (LMSW) to join our Social Work Department. This role involves providing comprehensive social work services to support patient discharge planning and facilitate smooth transitions to appropriate care settings. The ideal candidate will have recent hospital experience and a deep understanding of social services within an acute medical inpatient setting.
Roles & Responsibilities:
Discharge Planning: Participate in discharge planning, develop and implement social care plans, and conduct resident assessments.
Administrative Duties: Complete medical forms, reports, evaluations, studies, charting, and arrange transportation as needed. Coordinate activities with other departments.
Interdisciplinary Team Meetings: Participate in interdisciplinary team meetings to discuss patient care and progress.
Patient Transition: Promote the optimal transition of patients for safe and appropriate discharge placements. Maintain ongoing communication with patients and responsible parties regarding progress and status.
Financial Assistance: Provide information to patients and families about Medicare/Medicaid and other financial assistance programs.
Consultation: Offer consultation to staff on psychosocial concerns, discharge planning, and placement after skilled nursing units (SNU) or acute rehabilitation units (ARU).
Regulatory Compliance: Demonstrate understanding of rules and regulations governing SNU and ARU.
Professionalism: Show autonomy, professional judgment, responsibility, and reliability in all tasks.
Reporting: Provide written and/or oral reports on social service programs and activities as directed. Conduct grievance tracking and report in meetings, evaluating and implementing recommendations related to social services.
Quality Programs: Engage in quality programs for SNU/ARU and demonstrate flexibility to meet patient and family needs.
Confidentiality: Maintain required department confidentiality and adhere to HIPAA regulations.
Patient Care: Deliver competent patient care, interpreting data appropriately and adapting procedures based on age-specific growth and development.
Professional Growth: Engage in personal and professional growth to enhance job functions and participate in departmental education programs.
Mission Alignment: Communicate and perform duties reflecting the organization’s Mission, Values, and Impact. Support and adhere to all medical center, departmental, and safety policies and procedures.
Safety Concerns: Proactively identify and address safety concerns in the work environment.
Additional Responsibilities: Perform other duties as requested by the supervisor, manager, or department head.
Minimum Qualifications:
Must possess a current LMSW with recent hospital experience within the last year.
Experience as a LMSW in an acute medical inpatient setting is required (this is not a behavioral health position).
LMSW or LCSW credential is required.
