Job Title: Social Worker (Palliative Care) Purpose of the Position: Provides support to patients who would benefit from psychosocial and emotionalintervention. Offers necessary community resources coordination to patients and families who needassistance with navigating the complex healthcare system.ResponsibilitiesCoordinate and provide home visits to patientsAdhere to HIPPA at all timesDocuments psycho-social assessment, patient status, and outcomes after each visitEnsure regulatory compliance with documentation requirementsAssist the palliative team and/or directly facilitates patient-family meetings for goal setting andcare planningProvides education and emotional support to patients and familiesProvides community resources and referrals as needed to patients/caregiversSupport patient and family through referral to hospiceParticipates in discharge planningParticipates quality improvement initiativesAttends and actively participates in IDT meetings, staff meetings, trainingsPerforms other duties as assignedJob Requirements Ability to travel per job requirements Thorough knowledge of community resources or desire/ability to obtain resources Knowledge of the psychosocial process in chronically and seriously ill patients and their families Capacity to relate and work well with others as a member of an interdisciplinary team; this mustinclude the ability to give and accept objective criticism constructively. Professional and courteousJob QualificationsMSW or BSW degree in social work from a school of social work accredited by the Council on SocialWork EducationMinimum of 2 years experience as social worker in healthcarePreferred experience as a social worker in community based care ie. Home Health, Hospice,Palliative Care.
