If yes, here's how you will contribute to the team: As a Home Health Medical Social Worker (MSW), you will provide critical support and resources to patients and their families in the comfort of their own homes. If you are a social worker with a passion for improving the quality of life for those in need, this may be the perfect opportunity for you! Your key responsibilities include: Provide emotional and practical support to patients and their families in their homesAssess patients' needs and develop individualized care plansLink patients with community resources and servicesCollaborate with interdisciplinary healthcare teams to provide holistic careMaintain accurate and up-to-date patient recordsStay current with advancements in home health care and social work practicesMust Have: Bachelor's or Master's degree in Social WorkAt least one year of Medical Social Work experience; home health experience preferredExperience in health care, home care or home health care settingsAbility to work independently and manage caseloadStrong understanding of health care systems and policiesProficiency in electronic medical records (EMR) systemsKnowledge of nursing practices, procedures, terminology, laws, regulations, and guidelinesAbility to effectively utilize EMR and other technology to maintain HIPAA privacy, continuity of care, and team communication. Skills & Attributes: Strong critical thinking skills when working independently in the field.Excellent interpersonal skills to build rapport with diverse patient populations.High level of organization and time management to navigate a daily route.Nice to Have: Experience in acute care and working in a home environment.Experience with KanTime EMR.
