Geriatrics PCP - Holistic, Low-Panel Care

As a Primary Care Physician on our interdisciplinary team, you’ll have the opportunity to form lasting, meaningful relationships with your patients and make a tangible difference in their lives every day. Unlike traditional primary care settings, our value-based PACE program empowers you to provide comprehensive, integrated care focused on quality — not volume. With a smaller patient panel, you’ll be able to dedicate the time and attention each participant deserves. What You’ll Do Conduct thorough, in-person physical and wellness assessments. Collaborate with the Interdisciplinary Team to design and implement individualized care plans. Coordinate participant transitions between acute, short-term, and long-term care settings. Focus exclusively on holistic care — no hospital rounds — with an average of eight patients daily. Requirements Qualifications Completion of an accredited residency program (ACGME required) MD or DO degree Board Certification preferred Active, unrestricted California state medical license State CDS certification and DEA registration Eligibility to participate in Medicare and Medi-Cal programs Specialties considered: Internal Medicine, Family Medicine and Geriatrics Benefits What We Offer Our Physicians We believe exceptional care starts with supporting the professionals who deliver it.
