The Supportive Housing program providesintensive wrap-around supports to individuals facing significant barriers toobtaining and maintaining stable housing, often with a history of behavioralhealth and/or substance use barriers. The ideal candidate will be comfortableworking with these populations. The Nurse Case Manager will often travel todeliver medical case management in the home in order to further reduce clients'barriers to accessing care.TheHIV Medical Case Manager will communicate with providers regarding the client'scare plan and the medical treatment plan. In addition, HIV Medical CaseManagers will provide services such as: treatment management and medicationadherence, HIV disease and side effects management, nutritional therapy, oralhealth interventions and referral, liver health and hepatitis diseasemanagement (acute and chronic) and HIV risk reduction. Essential Functions and Responsibilities:ClientManagement: Carry an active caseload of eligible clients, providing directone-on-one medical case management services. Able to provide servicesin-person, over the phone and on a drop-in basis. Assessments& Planning: Conduct health assessments and goal planning in collaborationwith the client to identify medication adherence plans and barriers to care.Develop individualized care plans based on assessments and available resourcesto improve medication adherence and overall health. Coordinationof Services: Work collaboratively with case management team, providers,community partners to facilitate access to HIV treatment and medical care.Provide client education pertaining to HIV and other medications, diseaseprogression, and risk reduction. Advocacy:Advocate for clients with medical service providers and case managers. Conductcase conferences with case managers and medical providers as needed. Documentation:Maintain accurate and timely documentation of client interactions, servicesprovided, and progress updates in agency databases. Process and collectnecessary eligibility documents including Authorization for Release ofInformation.Referrals:Refer clients to infectious disease providers and medical resources as neededand follow up. CommunityEngagement: Build partnerships with medical providers in our service region.Attend relevant community meetings and participate in ongoing training andprofessional development. Travel:Frequently travel across service area, which may encompass multiple counties toprovide in-person services, including weekly home and/or community visits. Abilityto transport clients. Standards:Comply with all applicable program standards, guidelines, laws andadministrative rules. Reporting:Assist management with all necessary reports. Abilityto work well with others, handle stress appropriately, have reliable attendanceAbility to use personal cell phone to run agency phone system software, MFA, etc.Othertasks as assigned, including training/cross-training other staff as neededRequired Qualifications:ValidOregon Registered Nursing licenseOneyear of experience providing clinical care or medical case management topatients with chronic disease or infectious diseaseAbilityto pass AIDS RN Certification within 24 monthsTBtest (provided at agency) ValidOregon driver's license, or valid out of state driver's license with the abilityto get an Oregon driver's license within 30 days of establishing residency inOregon, and a driving record sufficient to be covered by agency auto insurancepolicy.
