Hospital Liaison

CareCore at Mary Scott is seeking an experienced and motivated Hospital Liaison to support referral development and strengthen relationships within the Dayton healthcare community.This position is ideal for someone with skilled nursing, long-term care, or healthcare marketing experience who enjoys building relationships, working in the field, and helping patients transition smoothly into post-acute care services.The Hospital Liaison will work closely with facility leadership and report directly to regional business development leadership.Responsibilities: Build and maintain strong relationships with hospitals, case managers, discharge planners, physicians, assisted living facilities, and community partners Conduct regular hospital and community outreach throughout the Dayton market Coordinate with admissions and clinical teams to support referral and census growth goals Serve as a point of contact for referral sources and assist with communication throughout the admissions process Maintain knowledge of Medicare, Medicaid, managed care, and insurance processes Represent the facility at networking events, healthcare meetings, and community events Track referral activity and assist with growth strategies and market development Promote the services and reputation of CareCore at Mary Scott within the communityQualifications: Previous healthcare liaison, admissions, outreach, or SNF/LTC marketing experience required Knowledge of skilled nursing, rehabilitation, and post-acute care services strongly preferred Familiarity with hospital discharge planning and insurance authorization processes Strong communication, networking, and relationship-building skills Self-motivated with the ability to work independently and as part of a team Reliable transportation and ability to travel throughout the Dayton areaBenefits: Health, Dental, and Vision Insurance 401(k) Retirement Plan Paid Time Off and Paid Holidays Employee Discount Program Supportive leadership team and growth opportunitiesAbout CareCore at Mary Scott:CareCore at Mary Scott is a historic and vibrant skilled nursing and rehabilitation facility located in Dayton, Ohio.
