This full‐time role focuses on delivering high‐quality clinical care in a remote environment while collaborating with physicians, RNs, case managers, and interdisciplinary teams to diagnose, manage, and prevent acute and chronic conditions.The NP is responsible for Plan Medical Management, virtual health risk assessments, chronic disease management, remote triage, and care coordination. This position requires strong communication skills, clinical judgment, and the ability to deliver value‐based care through telehealth platforms while maintaining compliance with state regulations, clinical protocols, and collaborative practice agreements.
