Registered Nurse-PACT Coordinator

The Registered Nurse PACT Coordinator delivers professional practice using the nursing process assessment, diagnosis, outcome identification, planning, implementation, and evaluation with general supervision. Provides Veteran/patient driven care by optimizing the patient experience, satisfaction, and/or safety for assigned patients. Seeks knowledge and learning opportunities that support current nursing practice and self-evaluationThe Registered Nurse PACT Coordinator Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Serves in an administrative role as a Primary Care Patient Aligned Care Team (PACT) champion and coordinates local PACT implementation, training, communication, and sustainability of the PACT model. Actively promotes the PACT model within the Ambulatory Care setting, educates core PACT team members, identifies, and addresses communication and workflow challenges, collaborates with leadership and associated stakeholders, and schedules meetings among teams. Duties include, but are not limited to:Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.Focuses is on individual growth and development in outpatient PACT nursing practice with increased levels of self-direction.Brings primary care principles and theory with limited application of critical thinking skills.Interacts with and assists PACT members in achieving compliance with Veterans Affairs Central Office (VACO), Veteran Integrated Services Network (VISN), and local policies and directives.Assists teams to maintain national Primary Care standards using clinical practice guidelines and clinical pathways.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: 8:00 a.m. to 4:30 p.m. Monday to Friday, Subject to change based on agency needs.Telework: Not availableVirtual: This is not a virtual position.Relocation/Recruitment Incentives: Not authorizedPermanent Change of Station (PCS): Not authorized
