Registered Nurse-PACT Coordinator

The Registered Nurse PACT Coordinator delivers professional practice using the nursing process assessment, diagnosis, outcome identification, planning, implementation, and evaluation with general supervision. Provides Veteran/patient driven care by optimizing the patient experience, satisfaction, and/or safety for assigned patients. Seeks knowledge and learning opportunities that support current nursing practice and self-evaluationThe Registered Nurse PACT Coordinator Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Serves in an administrative role as a Primary Care Patient Aligned Care Team (PACT) champion and coordinates local PACT implementation, training, communication, and sustainability of the PACT model. Actively promotes the PACT model within the Ambulatory Care setting, educates core PACT team members, identifies, and addresses communication and workflow challenges, collaborates with leadership and associated stakeholders, and schedules meetings among teams.
