Medical Case Manager

Position SummaryThe Medical Case Manager provides ongoing medical case management for HIV positive patients, ensures access to appropriate services and coordinate service delivery. Emphasis is on treatment adherence, retention in care, and viral load suppression.Essential FunctionsComplete in-depth psychosocial assessment.Develop individualized treatment plan.Assist clients with completing health insurance and social service applications.Monitor chronic no-show patients for primary care, substance use, mental health, dental, and specialty appointments.Provide extensive follow-up encounters via face to face, phone, home visits, and other encounters with or on behalf of patients.Document all activities in CAREWare databaseQualificationsQualificationsMaster’s Degree from an accredited university and LMSW licensure (within six months of hire).Minimum two years demonstrated case management experience.Advanced data management and analytical skillsSkillsMotivated, organized, and detail-oriented. Strong interpersonal and communication skills (both verbal and written). Ability to multi-task and handle several medical and support service components at one time.Ability to meet deadlines and to function independently under dynamic circumstances is critical. Proficient computer skills, including experience with Microsoft Office applications – specifically Excel, Word, Access, and Outlook required.
