Job DescriptionJOB OVERVIEW• Job Title: Optometrist• Job Type: Locum Tenens• Location: Dover-Foxcroft, ME• Service Setting: Outpatient Clinic (FQHC)• Coverage Type: Clinical Only• Coverage Period: April 20, 2026 - Ongoing (6 months)• Clinical Shift Schedule: Tuesday - Friday, 8:00 AM - 6:00 PM (1-hour lunch)PATIENT INFORMATION• Patient Demographics: All ages• Patient Volume/Census: Approximately 16 patients per day• Case Mix: General optometry including eye exams, diagnosis of vision issues, prescribing corrective lenses, and treatment of eye conditionsFACILITY INFORMATION• EMR System: Athena• Office Equipment Available: Standard optometry equipment• Support Staff Available: 1 Optometric Technician, 1 Optician• Reason for Coverage: Supplemental CoveragePRIVILEGES & COMPLIANCE• Credentialing Timeline: Minimal (FQHC)COMPENSATION & BENEFITS• Rate: TBD• Travel, Lodging, and Malpractice Insurance: TBDREQUIRED PROCEDURES• Comprehensive eye exams• Vision testing and correction• Diagnosis and treatment of eye conditions• Prescription of corrective lenses and therapeutic medicationsJOB REQUIREMENTS• Licenses: Active state license preferred; willing to license• Board: National Board of Examiners in Optometry certification required• Other Qualifications: Must be comfortable with independent chartingDUTIES & RESPONSIBILITIES• Provide comprehensive eye care services• Diagnose and treat visual and ocular conditions• Prescribe corrective lenses and medications• Collaborate with clinic staff
