Job Summary: The Registered Nurse provides skilled nursing care, case management, and hospice care to geriatric patients facing end-of-life care in Snohomish County. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team, working with an average of 4-6 patients per day.Location:Everett, Washington, United StatesResponsibilities:- Provide skilled nursing care and case management to patients.- Conduct patient assessments and develop treatment plans.- Administer medications and treatments as prescribed.- Collaborate with the interdisciplinary team to ensure comprehensive patient care.- Complete documentation by 9 AM the following morning.- Travel within a 60-mile radius from the hub/office to visit patients.Required Skills & Certifications:- Active Registered Nurse license.- BLS certification.- Minimum of 1 year of recent experience.- Experience with HCHB EMR.- Clinical Assessment, Medication Administration, and Treatment Planning skills- Interdisciplinary Team Collaboration skills- Excellent communication and interpersonal skills- Ability to work effectively in a fast-paced environment- Bachelor's degree in Nursing or related field- Driver's license, car insurance, and automobile for field workPreferred Skills & Certifications:- Hospice care experience.Special Considerations:- Weekend coverage may be needed, to be discussed during the interview.- No jeans allowed in dress code; scrubs of any color are acceptable.- Must use provided iPad for documentation; personal cell phone is not to be used for work purposes.Scheduling:- Guaranteed 40 hours per week.- Open schedule options: Monday-Friday, Tuesday-Saturday, or Wednesday-Sunday.
