Job TypeFull-timeDescriptionEastern Area Agency on Aging is seeking an Aging and Disability Center Resource Specialist to join our Community Services team. The Resource Specialist is a full time position at 35 hours per week. It is a dynamic and rewarding role for helping older adults, individuals with disabilities, and caregivers navigate complex service systems, community resources, and benefit programs. As an Resource Specialist, you will act as a vital link, presenting all available options to consumers so they can make independent, informed decisions about their health and independence.As an ADRC Specialist, you will:Screen & Assess: Conduct effective screenings (primarily by phone, but also in person) to identify needs, provide options, and assist consumers in accessing services.Navigate Benefits: Explain programs, benefits, and potential waitlists; provide necessary applications and forms to help consumers secure the support they need.Coordinate Care: Link consumers to in-home or community-based resources, often "thinking outside the box" to find informal or non-publicly funded solutions.Manage Data: Accurately record all client demographics and referrals in the database in accordance with federal requirements.Collaborate & Advocate: Consult with staff across all departments and programs to provide holistic support and develop informal support networks within the community.Professional Growth: Once eligible, complete all training requirements for the InformUSA CRS-A/D certification.What We're Looking ForExperience: At least 1 year of direct, hands-on experience in case management, social services, care coordination, or a related non-profit role.Education: High School Diploma or GED required; Obtain InformUSA CRS-A/D Certification (paid by the agency).Skills: Strong problem-solving, mediation, and negotiation skills.
