Shift: Day 4x10-Hour (11:30 - 22:00) Day 4x10-Hour (08:30 - 19:00) Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. Your expertise will help ensure patients receive personalized, effective treatment while supporting their comfort and well-being. Duties Administer medications, including injections and IV infusions, following physician orders and safety protocols. Conduct comprehensive patient assessments by monitoring vital signs, performing physical examinations, and evaluating symptoms to inform treatment plans. Assist with complex procedures such as catheterization, wound suturing, and tube feeding, ensuring aseptic techniques are maintained at all times. Manage patient documentation accurately within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments in memory or hospice care settings. Support acute pain management strategies and participate in triage to prioritize patient needs effectively. Collaborate with interdisciplinary teams in diverse environments such as ICU, emergency room, medical-surgical units, or outpatient clinics to deliver seamless patient care. Experience Valid LPN/LVN license in good standing with relevant state licensing boards. Prior experience working in critical care settings such as ICU or Level I Trauma Centers is highly preferred. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation. Experience providing care in various environments including hospital medicine, long-term care facilities, assisted living communities, or home health settings. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding, and medical terminology to support precise documentation and billing processes. Hands-on experience with IV insertion, phlebotomy, medication administration, and infection control protocols. Strong understanding of physiology, anatomy, and diagnostic evaluation techniques to perform thorough patient assessments and support treatment plans effectively.
