Joining this team means being part of a mission-driven organization that is growing, investing in its people, and expanding access to dignified care throughout the region.WHAT YOU'LL DOGrow meaningful referral networks - Build and maintain relationships with physicians, hospitalists, discharge planners, case managers, SNFs, acute rehab centers, and hospital systems.Be the face of our brand - Represent our home health and hospice services at community events, health fairs, and professional meetings.Educate and advocate - Lead in-service presentations that help referral sources understand our full scope of care, eligibility criteria, and care transition capabilities.Drive census growth - Execute strategic territory plans that consistently grow referral volume across both service lines.Collaborate cross-functionally - Work hand-in-hand with our admissions coordinators and clinical team to ensure seamless referral-to-admission experiences.WHAT WE'RE LOOKING FOR2+ years of sales, marketing, or business development experience in home health, hospice, or a related healthcare settingAn established referral network - you know the physicians, case managers, and SNF contacts in your territoryProven track record of meeting and exceeding census growth targets in a competitive marketStrong communicator - you can translate clinical concepts for non-clinical audiences effortlesslySelf-starter mentality with excellent time management across a large, diverse territoryFamiliarity with Medicare home health and hospice eligibility preferredValid driver's license and reliable transportation requiredWHY JOIN USAt the heart of everything we do is a deep commitment to the patients and families we serve - and to the team members who make it possible.
