This role is ideal for a nurse who values meaningful patient relationships, strong teamwork, and clinical autonomy while serving patients across multiple counties.The Hospice RN will act as a clinical case manager, collaborate with the interdisciplinary group (IDG), and support patients and families through thoughtful, patient‑centered care.Schedule & Work DetailsShift: 8:00 AM - 4:30 PMOn‑Call / Weekends: NegotiableMileage Reimbursement: Yes — reimbursed between visits, from first to last patient of the dayDress Code: Non‑specific (scrubs permitted)Key ResponsibilitiesProvide comprehensive hospice nursing care to patients in their homes, focusing on comfort, dignity, and quality of lifePerform ongoing nursing assessments and identify changes in patient conditionManage pain, symptoms, and medication regimens in alignment with hospice care plansServe as case manager for assigned patients, coordinating care across the interdisciplinary teamParticipate actively in IDG (Interdisciplinary Group) meetings, contributing to care planning and patient updatesEducate patients and family members on disease progression, symptom management, and end‑of‑life expectationsAssist with emotional support and guidance for patients and caregivers throughout the hospice journeyComplete accurate, timely clinical documentation in Homecare Homebase (HCHB)Travel daily between patient residences across assigned countiesRequired QualificationsRegistered Nurse (RN)Minimum of an Associate Degree in Nursing (ADN)Experience and working knowledge of Homecare Homebase (HCHB)Strong communication skills and compassionate bedside mannerAbility to work independently in a field‑based rolePreferred Skills & AttributesExperience performing hospice needs assessments and admissionsStrong clinical assessment and case management skillsExcellent organizational and time‑management abilitiesComfort collaborating with interdisciplinary teamsCalm, empathetic presence when supporting patients and families at end of life#PHC
