Healthcare Liaison – Skilled Nursing Facility

We are seeking an experienced and motivated Healthcare Liaison to support referral development and census growth for our Skilled Nursing Facility. This role focuses on building hospital relationships, coordinating referrals, and serving as a key connection between referral sources, facility leadership, residents, and families. Key Responsibilities Develop and maintain relationships with hospitals, case managers, physicians, and community partners Generate referrals and support census growth Serve as primary point of contact for referral sources Coordinate with Admissions, Clinical, and Business Office teams for appropriate placements Assist with intake information, insurance verification, and authorizations as needed Conduct facility tours and communicate clearly with residents and families Track referral activity and provide regular census updates Represent the facility professionally in the community Ensure compliance with federal, state, and payer requirements Qualifications Previous Liaison, Admissions, Marketing, or Business Development experience in Skilled Nursing / Long-Term Care REQUIRED Strong knowledge of Medicare, Medicaid, and managed care Existing hospital referral relationships strongly preferred Excellent communication and relationship-building skills Self-motivated and comfortable working independently Ability to manage multiple priorities in a fast-paced environment What We Offer Competitive compensation Supportive leadership team Stable work environment Opportunity to directly impact census growth and resident care Job Type: Full-time Pay: $80,000.00 - $100,000.00 per year Benefits: Dental insurance Health insurance Vision insurance Application Question(s): What are your salary expectations?
