Home Care Coordinator

Parx is looking for a Home Care Coordinator to join our team! The Home Care coordinator will be instrumental in improving the quality of life for those requiring non-skilled care while allowing them to preserve their independence and dignity in their own homes. The Home Care coordinator evaluates, plans, and coordinates to result in a favorable health outcome for every client. This role is responsible for ensuring that clients receive the appropriate care tailored to their individual needs, coordinating services, and acting as the liaison between clients, families, healthcare providers, and agency staff. The Care Coordinator will also be responsible for maintaining accurate records, assessing clients' needs, and ensuring compliance with Medicaid regulations.
