Supervisor Care Transitions

Job Summary As Supervisor, provides day-to-day operations support and supervision to the Care Transition Specialists; leading and coordinating referral through-puts while challenging the team to achieve excellence. Responsible for consultant service delivery. As Consultant, obtains and prompts conversion of referrals to admissions and facilitates a smooth transition of patients into Continuing Care facilities and Homecare services. Maintains communication with referral sources, external case managers and insurance companies, and assists with the coordination of services. Provides input to the Senior Director regarding budget, referral/admission barriers and strategies, staffing and customer service. Establishes, monitors and evaluates the performance goals of the liaisons. Encourages individual and team growth among the staff while providing high quality, compassionate service. Must be capable of traveling to and from referral sites located off campus. Essential Functions Provides day-to-day operations support and supervision to the Care Transition Specialists. Deployment of business development, sales, communication and marketing tactics. Establishes, monitors and evaluates the performance goals of the Care Transition Specialists. Encourages individual and team growth among the staff while providing high quality, compassionate service. Recruits, evaluates and hires staff as necessary. Leads referral process and staff work process development/continuous improvement.
