We are committed to compassionate, person-centered care that enhances quality of life.Position SummaryWe are seeking a dedicated and organized Care Coach to oversee and coordinate services for clients across counties in Indiana. This role involves regular client visitation, ongoing monitoring, incident management, and advocacy to ensure that each individual receives quality care, support in alignment with their Individualized Support Plan (ISP), and caregiver training.Key ResponsibilitiesConduct quarterly in-person visits to clients to assess well-being and service satisfaction.Conduct client’s home safety assessmentsComplete monthly check-ins, which may be virtual or in-person, as needed.Monitor the delivery of services to ensure compliance with client plans and waiver requirements.Document and report incidents, conduct thorough investigations, and assist in resolution and follow-up actions.Maintain accurate and timely documentation of all client interactions, service plans, and reports.Communicate effectively with clients, families, caregivers, and service providers.Ensure clients’ rights are respected and supported.Collaborate with the care team to update ISPs and address evolving needs.Travel throughout Indiana as needed to meet client and organizational needs.Train caregiversQualificationsA bachelor's degree in Social Work, Psychology, Human Services, or a related field is required.Minimum 1 year of experience in case management, preferably in home and community-based settings.Strong knowledge of Indiana Medicaid Waiver programs is a plus.Excellent communication, documentation, and interpersonal skills.Ability to work independently and manage a flexible travel schedule.Must possess a valid driver’s license and reliable transportation.Proficient with Microsoft Office and case documentation systems.Passion for advocating for individuals with disabilities and the aged.Work Environment & ExpectationsFrequent travel within IndianaHybrid work setting: remote administrative duties and in-field client visitsCommitment to ethical, person-centered supportIf you are passionate about making a difference in people's lives and are ready to join an amazing team at Kompassionate Care Services, we invite you to apply now!Job Type: Full-timeBenefits:401(k)Paid time offRetirement planAbility to Commute:Warren Park, IN 46219 (Required)Work Location: In person
