Core functions are outlined in the clinical privileges that correspond with this position.DutiesDUTIES Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practiceDevelops and implements an individualized plan to achieve optimal population health and disease managementSupports the needs of veterans through providing care on walk-in basic and/or coverage of paneled provider veterans as neededDemonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team membersMonitors and documents the effectiveness of treatment interventions - achievement of institutional quality measures - and performance measuresOrders and interprets labs - tests - and diagnostic studiesPrescribes pharmacologic and non-pharmacologic interventionsInitiates referrals and consults to specialty servicesProvides education to patients and their familiesAssignments may include management of specialty PACT such as Women's HealthTimely documentation of notes and encounter forms necessary in accordance with facility policy - all administrative work should be completed within a timely fashionAssesses and adjusts the plan of care as needed to achieve optimal patient outcomesExemplifies the ICARE core values of-Integrity - Commitment - Advocacy - Respect - and ExcellencePerform other duties as assignedVA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary - regular salary increases - potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave - 13 days of sick leave - 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: Monday-Friday from 7:30am to 4:00pm for four days a week9:30am to 6:00pm one day a weekTelework: Not Authorized Virtual: This is not a virtual positionRelocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not authorized PCS Appraised Value Offer (AVO): Not Authorized
