Regional MDS Coordinator (admissions) RN

We need a Regional MDS Coordinator to join our team at Carespring Healthcare Management in Loveland, OH. You will travel to our Ohio and Kentucky facilities assessing our new/existing patients to ensure all of the care needs are addressed and documented on the MDS. Must be a licensed RN in Ohio; prior experience in a long-term health care setting requiredPay Rate $36 to $44 Per HourGENERAL CONSIDERATIONS FOR THE POSITION:Must have current LPN or RN license for the state in which the facility is located.Needs to be flexible with her / his working schedule and able to work independentlyProfessional, organized, self-directed and knowledgeable with solid communication and teaching/training skills.Ability to positively interact and communicate with the leadership team in multiple facilities.Requires an understanding of the MDS process and a willingness to continue to learn and remain current with MDS updates set by CMS.Provide consultation, guidance, support, research, analysis, training, and expertise to attain business goals and quality outcomes in the delivery of care and services to our patients regardless of payer source.Obtain/Maintain current MDS Certification within 60 days of hire3 - 5 years MDS/RAI experience preferredDUTIES:Trains new MDS Coordinators/MDS Nurses in conducting resident assessments, developing plans of care, evaluating residents' responses to interventions, clinical records documentation, RAI manual and all applicable deadlines for resident assessments and completion of Minimum Data Sets (MDSs)Maintaining role of RN Resident Assessment Coordinator (RAC) or MDS Nurse when neededAssist with MDS Completion as neededProviding comprehensive training for systems related to the patient assessment instrument, clinical software, Medicare reimbursement, Medicare coverage/eligibility requirements, State Medicaid reimbursement systems, and special contract systemsMonitoring RAI process for accuracy and completenessWorking knowledge of PDPM and Case Mix in the state which the facility is locatedWorking knowledge of facility Admission process and assists with admissions as neededAssists facilities with surveys and survey preparednessMaintains ability to obtain and review QI/QM's. Works with IDT to initiate improvement projects to improve resident outcomesAttend's IDT Medicare meeting, PDPM, and Medicaid meeting to collaborate with the IDT to complete the MDS process.Conduct reviews to ensure supportive documentation is accurate according to MDS/RAI regulations.Travel required to various facilities.Provide education to licensed and non-licensed staffRN - 36 to $44 an hour, with shift diffCome join our team as a Regional admission/MDS nurse who assesses our new/existing patients to ensure all of the care needs are addressed and documented on the MDS.
