DESCRIPTION OF WORK This position provides professional social work services to support consumers with mental health needs and to guide their treatment, planning, and continuity of care. As a Social Worker 1, you will perform the following duties: Consumer Support: Provide age appropriate and culturally sensitive services that meet individual needs Resource Coordination: Link consumers with community systems, benefits, identification documents, and supportive programs Treatment Participation: Contribute information to treatment teams, complete assessments, and support discharge planning Regulation Compliance: Interpret and follow federal, state, and departmental regulations related to documentation and service delivery Family Communication: Establish connections with families, county liaisons, and community providers to support continuity of care Case Documentation: Prepare notes, reports, and updates to support treatment goals and maintain consistent care Interested in learning more? Work Schedule and Additional Information: Full-time employment Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. May change based on operational needs. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS Minimum Experience and Training Requirements: A bachelor's or master's degree in social work or social welfare. Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.
