This position works closely with an interprofessional care team to coordinate care and improve outcomes for a small panel of high-risk patients.This is a hybrid role (50% field-based, 50% remote) focused on longitudinal care management. Standard working hours are Monday through Friday, 8:00 AM–5:00 PM, with flexibility as needed to meet patient care needs.Duties And ResponsibilitiesServe as the primary social work contact for an assigned panel of high-risk patients, completing comprehensive psychosocial assessments. Collaborate closely with interprofessional care teams to deliver psychosocial interventions and case management services. Provide compassionate end-of-life care support for patients and families. Deliver longitudinal care management to support patient stability and improved outcomes. Build and maintain trusting relationships with patients to support effective care planning. Identify and address barriers to care through SDoH screening, referrals, and connection to community resources. RequirementsMaster’s degree in Social Work (MSW) or Licensed Clinical Social Worker (LCSW) in the state of Florida. Strong clinical judgment and case management expertise. Ability to meet Duo Health care team requirements, including background check, drug screening, and COVID-19 vaccination requirements. Reliable transportation with a verifiable good driving record; ability to travel to patient homes. Self-motivated with the ability to work independently while following established operating procedures. Demonstrated ability to build inclusive, equitable relationships with patients, caregivers, and colleagues. Experience working with diverse communities and community-based resources. Strong written and verbal communication skills. Comfort interacting with patients and care teams both telephonically and in person.
