This role ensures consistent, high‑quality community engagement, referral development, and education across multiple service areas. The Supervisor collaborates with internal teams and external partners to strengthen referral pathways, support strategic growth, and promote exceptional patient and family experiences.ResponsibilitiesSupervise Outreach Coordinators across multiple regions, providing coaching, performance feedback, and ongoing professional development.Oversee daily outreach operations, ensuring consistent execution of community engagement, education, and referral‑building activities.Implement and communicate departmental policies, procedures, and standards to ensure compliance and operational consistency.Monitor outreach metrics, referral trends, and engagement data to evaluate effectiveness and identify opportunities for improvement.Coordinate staff schedules based on program needs, community events, and strategic priorities.Build and maintain strong relationships with providers, facilities, community organizations, and other referral sources.Collaborate with Home Health, Hospice, Intake/Admissions, and Leadership to streamline referral processes and support service growth.Ensure accurate documentation and reporting of outreach encounters, partner interactions, and community engagement outcomes.Participate in short‑ and long‑range planning, quality improvement initiatives, and strategic outreach projects.Support budgeting activities related to outreach operations, events, and materials.Represent the organization at community events, health fairs, and professional meetings to promote Home Health and Hospice services.RequirementsAssociate degree required; two (2) years of leadership experience may be considered in lieu of degreeTwo (2) years of experience in Home Health, Hospice, community health outreach, care coordination, or related healthcare fieldPrevious experience and demonstrated skills in a supervisor or lead capacityKnowledge of Home Health and Hospice regulations, referral processes, and service models preferredDriver License/Abstract - Multiple IssuersOther: vehicle for work business activities Why MultiCare?Rooted in the local communityPartnering with patients, families and neighbors across the Pacific Northwest for more than 140 years.Growth and educationCompetitive tuition assistance, award-winning residencies, fellowships and career development to invest in you.Well-being and supportGenerous PTO, Code Lavender and Employee Assistance Programs to help you maintain balance and feel cared in your work and life.Living our valuesRespect, integrity, kindness and collaboration guide how we care for patients, communities and each other.Belonging for allResource Groups and outreach programs help ensure every team member feels safe, seen, heard and valued.Pacific Northwest lifestyleWork and live where natural beauty, adventure and strong community connections are part of everyday life.Pay and Benefit ExpectationsWe provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $94,889.60 - $136,572.80 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
