PRP Direct Service Worker LeadMonday - Friday 7am - 3pm or 6am - 2pm General SummaryThe PRP Direct Service Worker Lead performs the duties of a Direct Service Worker and additionally provides a number of administrative duties that support the coordination of services and PRP operations.  Direct Service Workers provide assessment and treatment services that promote recovery, full community integration, and improved quality of life for persons who have been diagnosed with a serious and chronic mental health condition that seriously impairs their ability to lead meaningful lives. Direct Service Workers are required to provide collaborative, person directed and individualized services that focuses on helping individuals develop skills and access resources needed to reduce or eliminate the maladaptive behaviors that stem from a mental health diagnosis. Principle Responsibilities and TasksTo perform this job successfully, the PRP Direct Service Worker Lead must be able to satisfactorily perform each essential function listed below:Serves as a liaison between the PRP and Mental Health (“MH”) Departments (at all CCG Sites) assuring that there is consistent collaboration and a constant flow of referralsCoordinates and processes referrals received from MH staff assuring that the PRP Screening, Letter of Acceptance and the Initial Authorization Request Form is completedCoordinates the referral process with the Revenue Cycle Staff and assures consistent communication between the two departmentsPerforms monthly chart reviews and completes documentation of the process and provides feedback to staff and leadershipAudits record keeping in SAMMS to assure that holds are being managed and that services and forms are completed and signedConducts the initial assessment to gain an understanding of the client’s level of functional impairments, strengths, abilities, needs and preferencesDevelops an individualized and patient centered treatment plan that describes the goals, objectives and interventions of the treatment processImplements the delivery of services through onsite (clinic setting) and offsite (home/community) services that includes:Teaching and InstructionEncouraging positive changeSkill demonstration and practicePromoting individual wellness, self-management and recoveryAssisting in the restoration and/or improvement of self-care and social skillsFacilitating effective strategies for strengthened independent living and the achievement of self-sufficiency goalsExposing individuals to academic, vocational and employment training and resourcesFacilitating culturally relevant activities directed toward improved social relationshipsProvides services through face to face interactions and telehealthPerforms documentation requirements related to treatment planning, monthly summaries, progress notes and other assigned documentsFacilitates monthly collaborations with the client’s assigned mental health therapist and treatment teamMaintains the clinical records and compiles documentation required by CCG and governing agencies including the signing of all forms and servicesParticipates in staff meetings, program clinical team meetings, clinical supervision sessions and in-service as requiredUnderstands and supports the CCG philosophy and treatment modalitiesEnsures confidentiality regarding client issues, background and treatmentExhibits clear, professional, respectful and effective communication when interfacing with families and external contactsSkilled in working independently and implementing time management strategiesExhibits a high degree of ethical conduct, integrity and confidentialityEnsures client information per HIPAA standardsMaintains case files within electronic health record (EHR)Performs other duties as assignedQualifications and RequirementsAt least a high school diploma or equivalent. Associates Degree or higher preferredPrevious experience working with emotionally and behaviorally challenged adults or experience working in a recovery-oriented programMust be experienced and proficient in using an electronic health record systemStrong interpersonal skillsAbility to travel in the local area and perform field workMust have validated driver’s license and a dependable vehicle to travel between sites and provide offsite servicesThis position must be actively working toward certification provided by the Commission on Rehabilitation Counseling
