OverviewThe Nurse Practitioner (NP) is a vital member of the clinical team, responsible fordelivering high-quality, patient-centered care in alignment with the organization’s standards and protocols. Working both independently and in collaboration with physicians, the NP performs comprehensive patient assessments, formulates diagnoses, and develops personalized treatment plans aimed at promoting long-term wellness. This role leverages advanced clinical tools, including AI-assisted technologies, to streamline clinical processes and enhance care delivery. Nurse Practitioners support patients throughout their healthcare journey, ensuring efficient clinic operations and improved healthcare outcomes.Roles & ResponsibilitiesClinical Duties● Conduct comprehensive patient assessments, including detailed medical histories,physical exams, and diagnostic evaluations.● Develop and implement evidence-based treatment plans for a wide range of acute andchronic conditions.● Order and interpret diagnostic tests, advanced imaging, and laboratory work to ensuretimely care.● Identify when specialist referrals are necessary and coordinate appropriate follow-up.● Provide essential patient education regarding diagnoses, medications, and preventivehealth strategies.Patient Experience & Workflow Optimization● Deliver exceptional care by actively listening to patient concerns and ensuring a positiveexperience.● Manage clinic flow efficiently to reduce wait times and adapt quickly to schedulechanges.● Partner with the virtual care team to conduct telemedicine visits, utilizing innovativetechnology to improve accessibility.● Collaborate with medical assistants, scribes, and aftercare teams to optimize the qualityand speed of patient care.Technology & Documentation● Accurately document all patient encounters within the EMR system.● Leverage AI copilots to assist with documentation, clinical decision-making support, andadministrative tasks.● Maintain timely and thorough records in accordance with organizational and regulatorystandards.Team Collaboration & Leadership● Work closely with physicians and clinical staff to foster a collaborative and professionalenvironment.● Provide mentorship and training to clinical staff, interns, and students to supportcontinuous professional development and high-quality team performance.● Contribute ideas for process improvements and participate in discussions to enhanceclinic efficiency.Other Duties● Actively participate in monthly provider syncs, one-on-one sessions, and company-widemeetings to ensure clinical alignment and seamless communication.● Complete company-assigned training to stay current with new services, operationalprocedures, and industry standards.● Maintain flexibility in scheduling to meet clinic needs, including evenings, weekends, andholidays.● Demonstrate professionalism and emotional resilience during high-stress or sensitiveclinical situations.Required Training, Certifications, or Credentials● Master’s Degree in Family Nurse Practice (FNP)from an accredited program.● Active New York State Certification and Licensure as a Nurse Practitioner.● National Board Certification (ANCC or AANP).● Valid DEA Registration.● BLS, ACLS, PALS, and Infection Control Certifications.● FMCSA Certification (must be obtained within three months of employment).● Suboxone and CDL Certifications (preferred/a plus).
