Jovenes is a place of personal transformation for homeless student. Our mission is to help homeless student and at-risk families become productive and integrated members of the community. We provide emergency and permanent housing options and other community based services that create opportunities for them to lead productive lives. We operate in Los Angeles and community East and South East of Los Angeles County. Jovenes, Inc. is committed, under the ‘College Success Initiative (CSI), to help homeless student achieve academic outcomes in their college careers. The goal of the program is to help students develop the appropriate connections and steps in life to become more self-sufficient and independent by accessing permanent housing and or while pursuing higher education. The agency is continuing to develop partnerships with local institutions to make sure that there is a strong support on and beyond campus for student in need. JOB TITLE: Program Coordinator – San Bernardino IMMEDIATE SUPERVISOR: College Success Program Director In partnership with San Bernardino Valley College (SBVC), the Jovenes Inc. agrees to provide housing assistance and co-located housing-related case management to achieve housing security for eligible students. Funding for this partnership is provided by the State of California, through Assembly Bill 74 (AB-74), College-Focused Rapid Rehousing. The primary responsibility of the Program Coordinatorwill be responsible for monitoring and coordinating the day to day program activities, and supervising several direct service support staff in delivering high quality and consistent services to students attending SBVC. As Lead Case Manager for SBVC, the Program Coordinator must also assess, provide case management, crisis intervention, and coordinate activities, including supervision of student housing for college students experiencing homelessness. The successful candidate will also be charged with arranging staff training sessions and developing the work schedules for staff. The Program Coordinator will also be assigned a case load that consists solely of homeless college students attending San Bernardino Valley College. DUTIES AND RESPONSIBILITIES Under the supervision of the Program Director, the position will: CASE MANAGEMENT & HOUSING OUTREACH Conduct outreach and process incoming referrals for student in need from SBVC Assess students for skills, challenges and appropriate services. Develop with student, Individualize Service Plan (ISP) that will be updated monthly. Provide students with referrals and monitor progress as needed in particular to mental health and substance abuse services and to opportunities for educational & career development and personal enrichment. Evaluate transportation needs and support them in developing a sustainable plan. Provide clear direction, support, and follow up to students. Conduct in-home and on campus case management meetings and client supervision Support students in identifying permanent housing options that would help them best reach their educational goals. Develop and maintain relationships with landlords for master-leased and rapid rehousing options Regularly update participant files and manage program database Deliver and develop program reports to program and agency leadership on a regular basis and when prompted Maintain files in a secure and locked cabinet Collect, prepare and submit student records regularly and when prompted Follow at all times the agency professional code of conduct. Develop appropriate procedures in case of crisis Coordinate other program staff in service delivery and facility monitoring. Delegate activities and follow up to other program staff to maximize clients’ progress. Other duties and responsibility may be developed by immediate supervisor according to program needs. PROGRAM COORDINATION Under the supervision of the Program Management, implement and or develop as needed, policies and procedure for all aspect of the program. Supervise activities, recruit, train, and delegate tasks to subordinate staff. Create crisis management plans that will be executed in the event of a problem with subordinate staff or a program participant Develop comprehensive reports on ongoing program participation and submit summary reports to management on a regular basis Coordinate a regular schedule of program activities for program participants in collaboration with other team members Coordinate and Co-Locate on San Bernardino Valley College campus to work with students and campus partner team Ensure the quality and organization of record keeping, data collection in HMIS, process flows, and service delivery by subordinate staff personnel. Equipment and Computer Software Skills and Abilities: · Computer literate (basic working knowledge of Microsoft WORD, Excel, and Outlook) · Able to use telephone, typewriter, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required EDUCATION, EXPERIENCE, CERTIFICATES & TRAINING Bachelor’s degree and 2-3 years of experience in case management, homeless services, social services or another similar setting; and 1-2 years supervising one or more employees. Experience working with transitional age student, individuals with mental disabilities, low income families. Demonstrated ability to work with a culturally diverse clientele & staff Valid California driver’s license and personal vehicle insurance carrier relating to clients, staff agency representatives, community members and others. This position will require you to drive your personal vehicle for work. Salary range for the position is $31.01-34/h. Level of experience and educational level will be taken into consideration.
