If you are passionate about making a positive impact in patients' lives and working in a supportive and family-oriented atmosphere, we encourage you to join us in providing exceptional home health care.Please note that this role is per diem with a rate of $110 - $120 per visit.ResponsibilitiesEvaluate patients' functional abilities and home environments to identify needs and safety concernsDevelop personalized treatment plans and communicate them to physicians and the care teamTeach and retrain patients in daily living skills affected by illness or injuryRecommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devicesMaintain accurate clinical records and promptly report changes in patient conditionCoordinate with the broader care team to ensure seamless, patient-centered careEducate family members and caregivers on the patient's treatment approachSupervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled serviceSupport discharge planning as patients transition out of home health carePerform other duties as required for patient servicesRequirements Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy AssociationRegistered with the National Registration Examination of the American Occupational Therapy AssociationActive California Occupational Therapy licenseMinimum 2 years of clinical OT experienceCurrent CPR certification with AHAValid driver's license, reliable insured vehicle, and the ability to travel to patient homesStrong communication and organizational skillsBenefits Flexible ScheduleAbility to Convert to Full-Time Based On Performance If InterestedPaid Sick DaysPaid Bereavement LeaveSkills Training & DevelopmentCompany Social EventsMileage Reimbursement for Patient to Patient VisitsReferral Bonus
